A Strategic Outline Case (SOC) to consider the options to address unscheduled acute care on the Bronglais site, taking into account some of the factors that will influence the configuration of services such as: - the commitment to provide 24 hour access - the interplay of acute diagnosis, stabilisation and treatment for unscheduled care, including acute assessment facilities - acute activity profiles - working practices and models of care, such as the role of GPs in emergency and chronic disease care - operational services: “back-end services” and the configuration of clinical teams - the integration of services, for example, in a “multi-agency crisis centre” - the use of existing facilities and physical reconfiguration on site The process will include: - development of a a clinical change process - using capital as an enabler of change - with other enablers, such as telemedicine and telecare
